Florence is helping with Diabetes and Heart Failure: Must see video

Date posted: 29 Feb 2016

NHS Western Isles share their experience with Florence in their ‘Working with Florence’ video. The video follows two patients, Erin and Chris along with Nurses Jane and Debra and their journeys with Florence.

The video illustrates how Florence supports existing path gateways, which have already been approved by a Clinician.

Erin was diagnosed with Diabetes 4 years ago and tended to disengage with her condition. She was very excited to be introduced to Florence and has now been texting her blood glucose twice a day for 6 weeks, as well as being prompted to take her regular injections. Erin has completely turned around her diabetes management since starting Florence. Erin has reduced her long-term blood glucose by a third, which has huge implications on her long-term health. She has also reduced her risk of kidney, cardiovascular and eye problems by 60%.

Chris is a heart failure patient and he uses Florence specifically to monitor his weight, which is a very important aspect of heart failure. His weight can help his Clinician track his fluid balance, the state of his heart failure and the medication he needs to be on. Chris talks about his first impressions of Florence in the video. He wasn’t sure of how the SMS system could help him, however he ended up being ‘quite surprised at actually how much it has helped me’. Within 6 weeks, Florence has become a part of his life. He feels reassured that someone is helping him to monitor his weight and that someone’s keeping an eye on him. Chris is now able to manage his own care resulting in less face-to-face time with his Clinician meaning less time off work.

These case studies show how Florence is changing lives.
